By filling out this form I am registering for classes at Nicole's Dance Center Elite.
The cost of the program is $100.00 due for registration deposit. The monthly payment is $89.45.
The program payment includes your registration fee, administrative fee, tuition from Aug.10th-June15th, recital costume, along with recital video.
If at any time you would like to stop taking classes a 30 day notice is to be given in writing.
No payments will be deducted after the disenrollment process it completed.
Payments will be taken at the studio the first day of class.
Monthly payments will be made on the 1st of each month.
I/We the legal guardians of the applicant, apply for enrollment in Nicole’s Dance Center Elite and hereby give my/our approval to participate in any and all class activities. I/We hereby waive, release, absolve, indemnify, and agree to hold harmless Nicole’s Dance Center Elite, it’s officers, employee’s for transporting my/our child to and from activities, or in activities for any claim arising out of any injury to my/our child whether the result of negligence or any other cause. I/We voluntarily and knowingly acknowledge, accept, and assume these risks.
